Eagles Wings Homes LLC provides person-centered Home and Community-Based Services (HCBS) designed to support individuals with intellectual and developmental disabilities in living safe, independent, and meaningful lives within their communities.

Our services are delivered in accordance with Indiana Bureau of Developmental Disabilities Services (BDDS) guidelines and are provided under approved waiver programs including the Community Integration and Habilitation (CIH) Waiver and the Family Supports Waiver (FSW).

Each service is coordinated through the individual’s Support Coordinator and is delivered according to the goals outlined in the individual’s Person-Centered Individualized Support Plan (PCISP).

Our goal is to empower individuals to develop life skills, strengthen independence, and actively participate in their communities.

Residential Habilitation and Supports

Residential Habilitation and Supports are services provided under the Community Integration and Habilitation (CIH) Waiver that assist individuals with intellectual and developmental disabilities in residential settings. These services may be provided on an hourly basis or up to a full 24-hour support schedule, depending on the individual’s approved level of care.

Our team provides structured assistance designed to ensure the health, safety, and overall well-being of the individual while promoting skill development and independence. Staff members assist with activities such as personal hygiene, meal preparation, medication reminders, household management, safety monitoring, and daily routine development.

Residential Habilitation services emphasize person-centered planning, meaning the individual’s goals, preferences, and abilities guide the services delivered. The goal is to create a safe, supportive environment where individuals can live with dignity while developing skills that allow them to thrive in their homes and communities.

Participant Assistance Care (PAC)

Participant Assistance Care (PAC) services are provided under the Family Supports Waiver (FSW) and are designed to help individuals remain safely in their own homes and communities while receiving support with essential daily activities.

PAC services provide hands-on assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, mobility assistance, meal preparation, and household tasks. Staff members also help monitor the individual’s health and safety while encouraging independence and participation in community life.

These services enable individuals to maintain their independence while receiving the level of assistance needed to safely manage daily routines. PAC services focus on promoting dignity, autonomy, and improved quality of life.

Day Habilitation

Day Habilitation services provide structured opportunities for individuals to develop life skills and social connections in community-based settings outside the home.

Participants engage in activities that support development in areas such as communication skills, self-care, community integration, sensory and motor development, and daily living skills. These programs are designed to help individuals build confidence and develop the abilities necessary to function more independently.

Day Habilitation services emphasize social engagement, skill-building, and community inclusion, providing individuals with opportunities to participate in meaningful activities that enhance their independence and quality of life.

Respite Services

Respite Services provide temporary relief for caregivers while ensuring individuals continue to receive safe, supportive care. These services help maintain the stability of caregiving relationships by allowing families and primary caregivers time to rest, attend to personal responsibilities, or address emergencies.

Respite care may be provided in the individual’s home, a provider-approved location, or another setting depending on the individual’s needs and approved service plan.

The purpose of respite services is to maintain continuity of care while supporting the long- term well-being of both the individual and their caregivers.

Extended Services

Extended Services provide continued support for individuals who require ongoing assistance to maintain the progress achieved through habilitative programs.

These services reinforce previously learned life skills, support community engagement, and help individuals maintain stability in their daily routines. Staff members provide structured supervision and guidance designed to promote long-term independence and sustained success in community living.

Extended Services ensure individuals continue receiving the support necessary to maintain independence and improve overall quality of life.

Transportation Services

Transportation Services assist individuals in accessing essential resources and activities within their communities. These services support travel to medical appointments, employment opportunities, educational programs, recreational activities, and other services identified in the individual’s service plan.

Reliable transportation helps individuals remain engaged in their communities while maintaining independence. Our transportation services prioritize safety, reliability, and accessibility, ensuring individuals can participate fully in community life.
